In January 2016 NHS England appointed Warrington Health Plus as caretaker managers of the former CCA Partnership practices at Chapelford, Culcheth and Appleton. These practices are now known as Health Plus Practices - you may have noticed our name change.

Warrington Health Plus will be responsible for managing Culcheth, Chapelford and Appleton Medical Practices until the end of March 2017.

Under this contract, Warrintgon Health Plus plan to further embed the practices into their clusters with a focus on developing services for a sustainable future. Health Plus Practices will be developed to create a model of excellence which other practices can learn from.

Warrington Health Plus will be working closely with practice staff and patients to continue the high quality service provided at these practices. Patients shouldn't notice any immediate changes to their practice - it will continue to be business as usual.

Each practice's Patient Participation Group (PPG) will also be involved in the development of the practices as part of our commitment to putting the patient first.

This is an exciting time for Health Plus Practices and we hope you will support us as we work to improve our services, just as we look forward to continuing our work for you.
